Regarding Firefox and the cache, there's a Web Developer's Toolbar plug in for Firefox. I recommend it to any developer...lots of cool tools.
It has an option to clear the cache: Miscellaneious -> Clear Private Data -> Cache
It asks for confirmation, but in the confirmation you can select the option to not ask for confirmation in the future. I use it repeatedly when configuring FCKEDitor. Change FckEditor config, clear cache and Ctrl-Shirt-R to force a reload.
Take a look at the source of the page. Is the toolbar loaded? The source code should say something about an iframe, ending with something like "?InstanceName=text&Toolbar=Default" Find the file fckconfig.js and search which toolbar is loaded and if the toolbar exists.
