Image upload in php isn't working correctly

Everything claims to be a success when i upload an image using fckeditor, but the image is never actually uploaded. It places a img tag in with what would be the correct path, but a red X appears.

Am I missing something in my config or file/folder permissions?

Using FCKeditor for php 2.4.3
