By default CKEditor works on HTML code. It can be customized to work with bbcode, but it requires some work on the developer's part, there is no out-of-the-box solution for that. See the "Output for BBCode" (bbcode.html) sample from the "_samples" folder.
Thanks for reply, but, that example is for nothing I alredy got working ckeditor at my board, but i can't add code button (if i add it in way described in example, it will not show on posting page, but others are ok ). Note: That board is using ckeditor through mod from moddb: http://www.phpbb.com/customise/db/mod/ckeditor/
I think I'm having similar problems. I've configured CKEditor with the BBCode plugin, but some buttons, such as "Underline" and "Smiley" just won't show up. Any ideas?

Answer for upstairs
I ran into the same problem as Peet and I find why!
The reason that the button manually added won't show up is because I download the standard version of ckeditor from the download page.
Download the full version of ckeditor, then the buttons will show up. Problem solved.
Thanks! Actually, I had
Thanks! Actually, I had figured this out, but until you posted and the system emailed me I couldn't find this thread to say so. @'.'@
