I am using FCKeditor to teach grammar at http://www.colorcodedgrammar.com/editor/_samples/default.html. I teach grammar by color-coding it. I use span tags which apply the CSS border-bottom property to color-code text for display on a web browser and for printing. Because my system uses secondary underlines, I must nest span tags. Each span tag adds an underline beneath the text. Secondary underlines must be separated by a white (invisible) underline in order to prevent the colors from blending. Each additional underline requires two span tags.
I only need a few of the tools on the default toolbar. The most important tool for me is obviously the Style menu. I have added my 21 color-styles to the Style menu along with a white spacer style. They apply style rules with a class attribute. The white spacer span tag applies style rules with an id attribute. I have also modified the Size menu to add span tags that use a style attribute to apply inline style rules. The editor selects tags based on the attribute type, so my nested tags must have different attributes.
I'm wondering how I could place multiple but separate instances of the style menu on the toolbar. I suspect that I need to declare new objects in the fcktoolbarstylecombo.js file, but I don't know how to do that. I would appreciate any suggestions.
I also need to add line-height style rules to multiple lines of text to increase their leading when I use more than three colored underlines. The more secondary style menus that I could add, the better the editor would work for me. However, if I could add some span tags with custom buttons, that would also work. Note that my coding technique is currently just for IE Explorer. Secondary levels can be displayed on FireFox if the margin-padding property is used. The problem with this coding technique occurs when three or four underlines need to be placed under words. The white underline separator is a simple, consistent way to separate text.

Re: Adding a second and a third Style menu
Can't you use an image background to mimic double or triple underlinings? Maybe increasing the bottom-padding or line-height.
Adding style dropdown should be possible, but will need custom coding...
