Image Upload Finds Mystery Duplicate File

I am encountering an odd issue. When trying to upload an image, we are receiving an message stating that a file exists and the file being uploaded will be renamed. The issue though is that the image does not exist on the server. I am sure that I am uploading a unique file. When viewing the directory on the server, I can see that there are no duplicate files.

Anyone have any thoughts as to why this is happening?

Are you using CKFinder? If so, which version and what is your server environment? Which browser do you use to upload the file? Does the renamed file show on the server after the upload?

I am using ckFinder version 2.0.2. We are using a Coldfusion 8 running on a windows server. This is occurring with both Chrome and Firefox. Yes, the file shows up on the server with the with the new name.
