Ad 1,3) You can do whatever you like with the PHP connector which is located inside of core\connector\php directory. Ad 2) Unfortunately there is no easy way to customize CKFinder interface, including the upload form (it is located in core/pages/ckffiles.html, it was not designed to be modified in any way, but if you want, you might try changing it).
