Making a Upload file button.

Hi,
I have set up a template to help display uploaded files see

I want to create a button like the insert/edit image button that will automatically apply the template to the file users specify. I'm not really sure where to start does this need to be done as a plugin? Can I just copy and modify the image button if so where is the code for this button keep?

Could someone just give me a few hints to get me started even if it just a link to appropriate documentation.
