there is one over at tinyMCE - maybe you can adapt it to your needs.

No mod, no plugin, no hack that easyli let you add a caption to a inserted image?

try this: http://kfm.verens.com/

you can add a caption to an image by right-clicking it (in the file manager) and clicking "edit comment".

the comment is then added as the Alt parameter of teh image. You can copy/paste it from there if you wish.

double-click the file you want (or click it, then hit Enter), in order to push it back to FCKeditor.
