I upgraded from CKeditor 4.2 to 4.2.1. Since I did this, I am having problems with the icons showing correctly. I have even installed a different skin and tried that but it has the same problems. I attached a screenshot that shows the issue.
As you can see the .cke_button__outdent_icon is listed twice. If I unchecked the top one, the bottom one will load and it will look correct. Does anyone know what could be going on here?

I've got to ask this - did you clear the cache? :). Normally CKEditor when upgraded works without problems, because the timestamp (used when loading assets) differs between versions. But in some cases it may happen that you changed e.g. preset or switched between two custom builds (within one version) and then timestamp is not enough.

And do you have the same
And do you have the same issue on samples attached to the CKEditor package? If so, then please create a bug report on http://dev.ckeditor.com and attach that package. If not, then you would have to try creating such sample on which we could reproduce this issue. Anyway, we don't have any bug report regarding this, so I guess that there are some specific conditions under which it doesn't work correctly.

I've just downloaded a standard package with Moono color skin and it works for me. So that's not a sufficient requirement :).

Yup - we're working on eliminating this problem, because the same timestamp is used in both packages (because it's related to CKEditor version, not CKEditor's build configuration).
